KELLY John Boatswain : Failure to keep proper lookout during watch

LARSEN Paul Portoculus Skipper : Stranding and loss due to failure to navigate with proper and seamanlike care. Certificate suspended 9 months, with recommendation to grant lower certificate.

PETTMAN Joseph Mate : In grave fault, failure to keep proper lookout during watch. Mate not asleep during watch but failed to see that the vessel was running straight to land. Certificate suspended 6 months, with recommendation to grant lower certificate
